IMPORTANT INFORMATION PERTAINING TO REGISTRATION AS A PROSPECTIVE SUPPLIER:

SUPPLIER REGITRATON FORM This form needs to be completed by ALL vendors seeking registration as anapproved supplier. All fields of the questionnaire MUST be completed in full. The balanced scorecard MUST be completed, printed, signed and posted together with the original valid Tax Clearance Certificate and the Entity Maintenance (EFT payment stamped by the bank) form to the following address: Tender Box, First Floor East Tower Momentum Building C/o Prinsloo and Pretorius Streets Pretoria, 0001
The above information must be submitted in a sealed envelope marked clearly with the words: SUPPLIER REGISTRATION DETAILS CATEGORY APPLYING FOR: ____________________ (see categories in Advertisement) and must reach this address by no later than 24 May 2005 at 11h00 Applicants will be contacted via fax and must therefore submit an operational fax number, failure to comply will result in excluding the supplier from the database. Suppliers will not be notified of the outcome but will be advised of the outcome if telephonically requested. The DoJ&CD reserves the right to accept or reject any application without being obliged to give any reasons in this respect. Suppliers must comply with all the registration criteria for registration to befinalised. Failure to comply with any of the above may result in an application being declined.
